One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. We are currently seeking a 1 FTE System Navigator- Team Care as a valuable member of the Community Health Centre team, will provide support for clients with complex and multiple needs in the communities that we serve.
The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefits package.
The salary range for this position is from $53,897 to $64,932 per year. We currently do not use AI to screen resumes.
Qualifications: • Honours Bachelor degree in Social Work (HBSW) or Registered Practical Nurse diploma (RPN). • Minimum one-year work experience. • ASIST Training • Ability to effectively engage clients in the assessment and planning with regards to their needs. • Demonstrated ability to facilitate and advocate for services to support clients in their care. • Ability to work both independently and collaboratively with an interdisciplinary team. • Strong writing and documentation skills • A strong understanding of the determinants of health framework. • Strong understanding and knowledge of health services and community supports. • Ability to work flexible hours. • Ability to work effectively with marginalized and under serviced population. • Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured) • Awareness and sensitivity to socio-economic and cultural issues. • A current driver’s license and access to a reliable vehicle.
