Our work depends on aCohort & Capability Transition Managerjoining our team to support the Indian Health Service (IHS) Electronic Health Records Modernization (EHRM) program. AstheCohort & Capability Transition Managersupporting the IHS EHRM program, you willbe responsible forsupporting the smooth transition to Sustainment for incoming PATH EHR cohorts and new capabilities (products). You willpartner with other workstreams to effectively plan for and execute these transitions, accounting for any deviations that mayimpactSustainment, inclusive of new products,featuresand functionalities.
This role requires you to obtain andmaintainan in-depth Public Trust Level 5. This investigation will review personal and criminal behavior, financial conduct, foreign influence, as well as other adjudications.
HOW ASUSTAINMENT COHORT & CAPABILITY TRANSITION MANAGERWILL MAKE AN IMPACT:
Coordinate acrossinternal clientteams andpartners toensure cohorts and new product capabilities are transitioned to sustainmentefficiently
Identify, propose,and execute processimprovements where appropriate.
Ensuresappropriate tasksare completed on schedule.
Serve in a consultative capacity to the client and partners as a subject matter expert.
Improves operational management systems, processes, and best practices.
Creates and/ormaintainsstandard operating procedures inappropriate tooling.
Perform administrative and analytical tasks in support of contract program as needed.
Compile andorcontribute to weekly status meeting minutes, monthly progress reports, and monthly financial reportson behalf of your function as needed.
REQUIRED QUALIFICATIONS AND EXPERIENCE:
Bachelor’s degree or an equivalent combination of education and relevant experience.
Training, education, or hands‑on experience in task coordination, with a functional understanding of IT project management methodologies, tools, and best practices.
Experience proposing and executing operational improvements to existing projects/programs.
Task management experience and/or skills.
Willingness and ability to travel up to 25% of the time, as required.
DESIRED QUALIFICATIONS AND EXPERIENCE:
Ability to manage work in a variety of systems, including Jira and Confluence
Excellent organizational and time management skills; ability to managefrequentlychanging priorities of competing importance.
Ability to communicate and interact effectively with internal/external teams including keypartners and leaders
Ability to work independently with minimal supervision and within tight deadlines, following detailed written policies, processes, procedures, and work instructions.
Ability to produce high-quality documentation that contributes to the overall success of our program.

The likely salary range for this position is $127,500 - $172,500. This is not, however, a guarantee of compensation or salary. Rather, salary will be set based on experience, geographic location and possibly contractual requirements and could fall outside of this range.
