Chosen Family fosters a community-centred ecosystem that embraces diversity, flexibility, and enrichment for all. We continuously reimagine how our services can create platforms for equality, inclusion, and belonging. Committed to a culture of acceptance, we ensure that everyone is welcomed and valued.
Recognising the interconnectedness of our humanity, we strive to empower individuals to live independently. By honouring each person’s unique identity and experiences, we celebrate and uplift one another in return.
We are seeking a dedicated, compassionate, and patient Disability Support Worker to provide ongoing support to a young participant in Canton Beach, NSW.
The participant is a young person with Autism Spectrum Disorder (ASD) who is independently mobile and does not require manual handling, feeding assistance, mobility equipment, or support with behavioural concerns.
This role is ideal for someone who is patient, respectful, adaptable, and willing to take the time to build a genuine and trusting relationship with the participant. As this is the participant’s first NDIS plan, their routines, interests, preferences, and support needs are still being explored.
The successful applicant will be comfortable taking a flexible, person-centred approach and developing the support relationship together with the participant.
A female support worker is preferred for this role, based on the participant’s support preferences. We are particularly interested in applicants who are calm, approachable, patient, and comfortable supporting a young person to build confidence and independence.
The support will involve community access and household support.
At Chosen Family, we believe support is about more than completing tasks. It is about building trust, creating meaningful connections, promoting independence, and supporting people to live life on their own terms.
If you are a compassionate female Disability Support Worker who would enjoy building a meaningful connection with a young participant and supporting them to explore their interests, community, and independence, we would love to hear from you.
Join our team of dedicated Disability Support Workers and make a positive difference in the life of a participant.
We strongly encourage applications from people with disability, Aboriginal and Torres Strait Islander people, Culturally and Linguistically Diverse people, and LGBTQIA+ people. We welcome and value lived experience and knowledge. If you are comfortable doing so, please let us know in your cover letter if this applies to you.

Chosen Family is an inclusive, trauma informed NDIS provider delivering person centred support across social and community participation, support coordination, and psychology services.
We work alongside participants to understand their goals, experiences, and everyday needs, and design support that reflects real life, not just a plan on paper. Our approach is practical, flexible, and grounded in building meaningful outcomes over time.
Our services include community participation, daily living support, support coordination, and access to psychology services. We support individuals to build independence, strengthen relationships, and reconnect with their communities in ways that feel safe, comfortable, and aligned with who they are.
Chosen Family operates through a wraparound support model, where support workers, coordinators, and clinicians work together to provide consistent and connected care. This ensures participants are supported across all areas of life, rather than navigating services in isolation.
We have a strong focus on supporting individuals with complex needs, including those experiencing mental health challenges, trauma, or social isolation. Our team takes a respectful and understanding approach, ensuring support is always person centred and identity affirming.
We work closely with support coordinators, plan managers, allied health professionals, and community organisations to build strong referral pathways and collaborative outcomes.
At Chosen Family, we believe support should feel clear, consistent, and genuinely helpful. Our focus is on building trust, improving everyday life, and supporting long term independence.
