Working within a multidisciplinary team, you will help public housing residents impacted by relocation to access health, social and community supports, ensuring they remain connected and supported throughout the move and in their new communities.
This rewarding role combines outreach, client engagement, service coordination, case management, and community connections to improve wellbeing and achieve positive relocation outcome
This position requires applicants to work onsite and does not have hybrid working arrangements.
