The Clinical Care Coordinator will collaborate with various stakeholders, including; patients, healthcare providers, care teams, facilities, and schemes to ensure that all patients who qualify for care coordination are adequately cared for through the entire patient journey. They will be an advocate for their patients and ensure that the care team effectively coordinates patient care from request to post-discharge.
They will also be responsible for monitoring and reviewing the patient’s biometrics and immediate wellbeing with continuous record keeping on multiple electronic platforms.
To assess and coordinate emergency vehicles or calls to ensure a timeous response to incidents logged on Computer Aided Dispatch (CAD).
To assist in receiving , prioritise and log emergency and non-emergency telephone calls and to determine the appropriate level of EMS assistance
Dispatch vehicles according to the CAD system
Assist in receiving, prioritise, logging, and processing all calls and alerts received
Provide basic contact centre administration
ESSENTIAL EDUCATION: Grade 12
DESIRED EDUCATION: BAA/ Level 3 first aid Certification
ESSENTIAL MININUM EXPERIENCE: Contact Centre / Emergency Contact Centre exposure
DESIRED EXPERIENCE: 1 – 3 years dispatching in an EMS contact centre
