Working Hours: 18.5 hours per week
Contract Type: Permanent
Salary: £25,877.80 - £34,240 per annum (pro rata)

About the role

We're looking for a compassionate and motivated Support & Housing Management Coordinator to join Waythrough as we mobilise the Housing, Health & Wellbeing Hospital Discharge Support Service across County Durham.

This is an exciting opportunity to make a real difference to people experiencing homelessness or at risk of homelessness by supporting safe, timely and sustainable hospital discharges. Working alongside health, housing and social care partners, you'll provide person-centred support that empowers individuals to secure and sustain accommodation, improve their wellbeing and build the skills and confidence to live independently.

You'll work with individuals both in hospital and within the community, completing comprehensive assessments, developing tailored support and risk management plans and coordinating support throughout their discharge journey. You'll work closely with partner agencies to ensure people receive the right support at the right time, helping to improve outcomes and reduce the likelihood of hospital readmission.

The role also includes housing management responsibilities, supporting people to understand and maintain their tenancy, assisting with housing-related issues such as rent and benefits, carrying out property visits and helping people access the services and opportunities they need to successfully sustain their accommodation.

About the service

The Housing, Health & Wellbeing Hospital Discharge Support Service aims to prevent individuals aged 18 and over from being discharged from hospital into street homelessness, while supporting safe, timely and sustainable discharges through coordinated in-hospital intervention and time-limited post-discharge support.

The service provides proactive, responsive and coordinated support through:

• Early identification of individuals who are homeless or at risk of homelessness.
• In-hospital assessment, advocacy and discharge coordination.
• Time-limited post-discharge support to help people sustain accommodation and reduce the risk of hospital readmission.

The service specifically supports individuals who:

• Are homeless or at risk of homelessness.
• Have support needs relating to drug and alcohol use.
• Are known to Adult Social Care services, including those with assessed care and support needs, those open to Adult Social Care teams, and those requiring coordinated discharge planning.

About you

We're looking for someone who is passionate about supporting people to overcome barriers and achieve positive, lasting change.

You'll have experience of working with people who have complex or multiple support needs and be confident building positive relationships while maintaining professional boundaries. You'll be committed to delivering person-centred support and enjoy working collaboratively with colleagues and partner organisations to achieve the best possible outcomes.

You'll also have:

• A Level 2 qualification in Health & Social Care, Housing or a related subject.
• Experience supporting people affected by homelessness, substance use, mental health or other complex needs.
• Experience of completing support, needs and risk assessments and developing person-centred support plans.
• An understanding of safeguarding, risk management and professional boundaries.
• Excellent communication and partnership working skills.
• The ability to manage your own workload and work effectively both independently and as part of a multidisciplinary team.
• A full UK driving licence and access to your own vehicle, as regular travel across County Durham is an essential requirement of the role.
