We provide accommodation for customers 18+ who are homeless or at risk of homelessness that have a diagnosed mental health condition.
By supporting our customers, you’ll also help make our service a great place to work, unleashing the potential of colleagues and delivering excellent outcomes.

What you’ll do
Work with a team of Support Coordinators to deliver a high-quality service that meets Home Group’s KPIs.
Support customers to build independence and achieve their goals under the guidance of your Senior Client Service Manager, whether this be understanding relationships to addressing substance abuse.
Assist customers with varied needs including mental health, housing, and complex behaviours and risk.
Create and update support plans and conduct risk assessments, helping customers with daily tasks like cooking, budgeting, and moving to on into the community.
Ensure properties are clean, safe, and welcoming, helping reduce voids.
Enjoy time for meaningful interactions and collaboration, no rushing your chats with customers.

You have
Experienced in creating person-centred support plans, risk assessments, and reviews.
Skilled in coordinating and assessing customer referrals.
Able to work independently, stay calm under pressure, and demonstrate resilience.
Confident challenging behaviours that don’t align with our values.
Empathetic and passionate about supporting customers in housing or care settings.

The practical bits
Toward Road is a 24/7 service with a rolling rota (including shifts on weekends and bank holidays, no night shifts however there are late shifts).
Occasional flexibility required, and we’ll offer flexibility in return.
Some lone working during morning or late shifts.
Basic Microsoft Word and Excel skills required.
Confident using technology for support plans, training, and collaboration.
Enhanced DBS check required (we cover the cost).
