You will be serving NDIS Participants in and around FRANKSTON (Victoria)

About the Role:
As a Support Coordinator, you will work alongside NDIS participants to turn their plans into action. You’ll build meaningful relationships, guide participants to make informed choices, and connect them with the right services. Your work will directly shape participants’ independence, confidence, and quality of life.

You will:

• Transform participant goals into clear, achievable plans.
• Connect participants with the right providers, supports, and community opportunities.
• Monitor progress, track outcomes, and ensure supports are delivered effectively.
• Build and maintain strong relationships with Local Area Coordinators (LACs), providers, and key stakeholders to enhance participant outcomes and drive referral pathways for LifeFul.
• Identify growth opportunities for participants and LifeFul, helping people thrive while expanding our impact.
• Uphold NDIS standards, safeguarding practices, and operational excellence while maintaining 85% billable hours weekly.

What We’re Looking For:

• Experience in case management, care coordination, or disability services (NDIS experience highly valued).
• Strong understanding of NDIS funding, participant rights, and service navigation.
• Proven ability to build and leverage stakeholder relationships (LACs, providers, community networks) to support participant outcomes and business growth.
• Excellent organisational, critical thinking, and problem-solving skills.
• Proficiency with Microsoft 365, CRM systems, and accurate case documentation.

Mandatory Requirements:

• Certificate III (or equivalent) in Individual Support / Disability Support.
• NDIS Worker Screening Check, Working with Children Check, First Aid & CPR Certificates.
• Valid Australian Driver’s License and vehicle insurance.
• Completion of NDIS Quality & Safeguards training modules.
• Right to work in Australia.
