The referenced pay range represents the minimum and maximum compensation for this job. Responsibilities:
The Social Worker Supervisor for Pediatric Inpatient Psychiatry is responsible for the coordination of continuity of care and discharge planning on that unit. This position also provides general oversight and supervises social work staff and the delivery of services on Pediatric Inpatient Psychiatry. As appropriate and in conjunction with the SW Manager for Inpatient Psychiatry, they may provide consultation support on other pediatric disposition cases in the system.
The Social Worker, Supervisor, in collaboration with the SW Manager for Inpatient Psychiatry develops and maintains administrative liaison with community agencies and referral sources, both internal and external to the Department to facilitate continuity of care, and timely discharge of patients from the inpatient service.
The social worker who occupies this position provides social work services to pediatric patients and their families while boarding for admission on Pediatric Inpatient Psychiatry. As a member of the interdisciplinary treatment team, the social worker participates in the development and implementation of the patient’s treatment and discharge plan. The social worker is responsible for providing psychosocial and family assessments, and for the overall coordination of the discharge planning process. The social worker provides individual, family, or group treatment that will contribute to the implementation of a comprehensive discharge plan. The social worker also provides social work treatment in response to patient/family psychosocial problems which may impede care and/or discharge. The social worker develops interagency relationships and mobilizes community resources on behalf of the patients and/or their families.
