Salary Range: $56,485.00 - $71,600.00 SalaryUnder the supervision of the Care Management Manager, the Care Management or Care Coordinator Supervisor is responsible to provide supervision and oversight of staff including training, performance management, managing work schedules, standards of quality, and exemplifying excellent customer service. Supervisors will be supporting teams comprised of, on average, 10-15 care managers or care coordinators.

Essential Duties
- Monitors activities of Care Management team to ensure that services are consistent across staff and compliant with departmental and organizational policies and procedures and Volume 8 regulations. Assumes care management duties in absence of care managers as necessary.
- Supervises care managers or care coordinators, -interviews, hires, trains, mentors, conducts performance evaluations and performance improvement plans to ensure the delivery of quality services.
- Responsible for assignment of clients and caseload management. Monitors work assigned to each team member to ensure caseload equity and compliance with documentation timeframes and expectations.
- Participates in departmental functions on performance management, quality assurance, Human Rights Committee, Critical Incidents and other projects as assigned.
- Acts as a resource to care managers or care coordinators and other departments for problem resolution as needed.
- Represents RMHS on stakeholder committees and meetings to maximize collaboration with organizations that serve individuals with needs related to aging and/or disabilities.
Knowledge, Skills and Abilities
- Ability to provide supervision and direction to a team of Care Managers and/or Care Coordinators.
- Comprehensive understanding of Medicaid Waiver and State-Funded I/DD programs.
- Experience with multiple computer systems, including Benefits Utilization System (BUS), Interchange, and Case Logic.
- Skilled in the use of Microsoft software programs.
- Detail oriented and well-organized.
- Knowledge and experience with service coordination or case management operations in regulated environment such as Medicaid.
- Analytic and decisive decision maker with the ability to prioritize and communicate to staff.
- Experience with the use of data and technology to support human service case management delivery model.
- Knowledge of issues related to intellectual, physical, or developmental disabilities, long term services and supports, family dynamics, community resources.
- Ability to communicate effectively orally and in writing, while maintain a high level of confidentiality.
Minimum

Qualifications
- Bachelors degree in human services or related field
- 3 years experience working with individuals with elderly, physically disabled, brain injury, mental health, and I/DD, with at least two (2) of those years in the provision of Case Management Services
Preferred

Qualifications
- One year of supervisory or progressively responsible experience.
- Masters degree in human services, social work, or related field.
