
Hillcrest Children & Family Center is seeking a highly motivated Care Coordinator to support the grant initiative serving individuals experiencing homelessness, with a primary focus on the Federal City population and surrounding community.
This position is a field-based outreach role (approximately 95% community outreach) focused on engaging individuals experiencing homelessness and connecting them to comprehensive wraparound services. The Care Coordinator will work directly with individuals struggling with Substance Use Disorders (SUD), including Opioid Use Disorder (OUD/MOUD), behavioral health challenges, and other barriers to stability.
The Care Coordinator plays a critical role in reducing service gaps by building trust, conducting outreach, coordinating care, and linking participants to treatment, housing resources, medical care, and supportive services
