
The Substance Use Navigator and Enhanced Care Coordinator (SUN & ECC) at AMC works together with medical providers to integrate behavioral health and primary care for our patients. Provides culturally and linguistically competent services for our community on behalf of AMC. The SUN & ECC works with patients who have a range of emotional, behavioral health, and substance use issues. Supports patients who may be experiencing life transitions, depression, anxiety, or need support in reaching health goals.
The SUN & ECC will provide same-day mental health “touches” or warm hand-offs to begin the process of engaging with patients to better support their health and wellness, as well as short-term counseling; group counseling; co-facilitating group medical visits with a PCP; care coordination; and case management services.
The SUN & ECC will also work with the Medication Assisted Treatment (MAT) team and provide support and counseling for those identified as having an Opiate Use Disorder and other Substance Use Disorders. The SUN & ECC will also screen for the need for referral to more intensive substance abuse services as needed.
Some Essential SUN Duties and Responsibilities Include:
· Ability to work on a multidisciplinary team in a primary care setting
· Knowledge of the principles, procedures, methods, and techniques of behavioral health service provision, including:
o Legal and ethical issues
o Involuntary hospitalization/ 5150 process
o CPS & APS reporting requirements
o Danger to self and Danger to Others reporting requirements
o Crisis resolution skills
o Brief and solution-focused therapy
o Substance abuse disorder diagnosis and treatment
Some Essential Enhanced Care Coordinator Duties and Responsibilities Include:
The SUN & ECC supports whole-person, trauma-informed care coordination for patients with complex medical, behavioral health, substance use, and social needs, consistent with CalAIM Enhanced Care Management principles.
Care Coordination and Patient Support:
· Coordinate care for patients with complex medical, behavioral health, and social needs, ensuring timely access to appropriate services and resources.
Advocacy and Resource Connection:
· Advocate for patients to obtain and maintain access to essential services and supports, including housing, food assistance, transportation, benefits enrollment, and behavioral health resources.
Requirements
Education/ Experience:
· FQHC or similar experience in a healthcare setting preferred
· Experience working with Medication Assisted treatment (MAT) and other substance use disorders required
· Experience providing Warm Hand-Offs preferred
· Knowledge of CBT/DBT, motivational interviewing techniques, mindfulness and other stress reduction strategies, behavioral health approaches to harm reduction model
· Requires 3+ years of case management experience in a behavioral healthcare setting;
· Certified Drug and Alcohol Counselor preferred
· Experience using EPIC, an electronic medical record, preferred
Licenses and Certifications:
· Bachelor’s degree in behavioral health, social services, psychology, community health, or related field required; Master’s degree preferred.
· Certification as a Drug and Alcohol Counselor preferred
· Licensure or registration in a behavioral health discipline preferred
