Pyramid Healthcare is seeking a compassionate and dedicated Substance Abuse Counselor to join our Assertive Community Treatment Team (ACTT) in Charlotte, NC. Our ACTT program delivers intensive, community-based behavioral health services to individuals living with serious and persistent mental illness (SPMI) and co-occurring substance use disorders.
Rather than waiting for clients to come to us, our multidisciplinary team meets individuals where they are—in their homes, shelters, workplaces, and throughout the community—to provide personalized, recovery-focused care that promotes stability, independence, and long-term success.

As the ACTT Substance Abuse Counselor, you will:
Provide substance use assessments, counseling, and recovery support in community settings
Deliver individual, family, and group counseling focused on co-occurring mental health and substance use disorders
Develop individualized treatment plans and recovery goals
Complete clinical documentation, treatment plans, NCTOPPS, LOCAs, discharge planning, and required authorizations within established timeframes
Coordinate care with psychiatrists, nurses, therapists, peer support specialists, families, and community providers
Assist clients with relapse prevention, harm reduction, and recovery planning
Provide crisis intervention and participate in the ACTT on-call rotation (weekday and rotating weekends)
Participate in interdisciplinary treatment team meetings and case consultations
Travel throughout the Charlotte area to meet clients in homes and community settings
Required:
LCAS, LCAS-A, or CADC certification (or ability to obtain required credential within 90 days of hire)
Qualified Professional (QP) status according to 10A NCAC 27G.0104
Valid North Carolina driver's license with an acceptable driving record
Ability to work independently in a community-based setting while collaborating with a multidisciplinary team
Strong documentation, communication, and organizational skills
