Job Description

The Step Down Navigator will assist in the design, plan, and implementation of programs designed to advance the marketing plans and services of the hospital.

Duties include but not limited to:

  • Attend All Treatment Team Meetings: Advocate for Step-Downs
  • Achieve 5% step downs (Next Step Program) per week based on average daily census. I.E. (average census for week= 80, 4 step downs expected.)
  • Meet with all patients / guardians that have OP Benefits according to their VOBs & give them an Invitation to PHP
  • Meet with Intake / Intake Director Daily to receive OP Referrals & F/U with every referral (minimum of 3x & document follow up) (Review EMTALA for PHP/IOP Referrals)
  • Attend Flash Daily to report on Step Down referrals. (Report on BD Dept. in case of DBD/OP Director Absence)
  • Communicate effectively with a variety of individuals and function calmly in situations, which require a high degree of sensitivity, tact, and diplomacy. Provide prompt, efficient, and responsive service.
  • Update outlook calendar daily with outline of daily tasks, prospecting, scheduled meetings, virtual meetings, and calls.
  • Complete the 8-hour shift required for the FT position.
  • Verify communication by calling, text, or email.
  • Provide exceptional service as front person of GBHI.
  • Communicate politely with everyone coming or out of GBHI.
  • Complete Outpatient UR & Discharge Planning duties as directed by Director.
  • Complete other duties as assigned.

Requirements

Education and/or Licensure – High School diploma/GED required. No required licensure. Must possess or obtain a valid CPR certification and be certified in facility approved verbal de-escalation and physical crisis management techniques within 30 days of hire and prior to completion of orientation required.

Experience – Three (3) years in healthcare, preferably in behavioral health required.

Additional Requirements – Bi-lingual English/Spanish preferred but not required.

Knowledge Skills and Abilities

  • Knowledgeable of patient rights and laws pertaining to mental health and regulations pertaining to psychiatric nursing and the treatment of the mentally and emotionally disturbed patients.
  • Displays basic knowledge of treatment procedures; interventions common to acute psychotic as well as non-violent crises intervention practice, medical and psychiatric emergency procedures.
  • Communicate effectively with a variety of individuals and function calmly in situations, which require a high degree of sensitivity, tact, and diplomacy. Provide prompt, efficient, and responsive service.
  • Ability to exercise appropriate judgment in answering questions and releasing information; analyze and project consequences of decisions and/or recommendations.
  • Excellent written and oral communication English skills; skills in facilitating and/or co-facilitating process-oriented and didactic groups.
  • Ability to read, analyze, and interpret general business materials, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, proposals and grant submissions.
  • Ability to effectively present information both electronically and in person and respond to questions from clients, customers and the general public.
  • Ability to solve practical problems and deal with a variety of variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions.
  • Ability to effectively plan sales strategy.
  • Ability to negotiate and build relationships.
  • Ability to work on multiple tasks or parts of tasks simultaneously to ensure timely completion of work activities.
