What You’ll Own:

• Complete
Start of Care (SOC) admissions

, delivering thorough in-home assessments and accurate OASIS documentation.
• Develop and initiate
individualized care plans

, ensuring each patient is set up for safe and effective home health services.

Identify risks, gaps, and service needs early

, coordinating the right disciplines and resources from the start.
• Communicate clearly with
providers, caregivers, and internal teams

to ensure alignment and continuity of care.
• Set the tone for
quality, compliance, and patient experience

from the very first visit.

What Sets You Apart:

• Active
Minnesota RN license

with strong clinical foundation.
• Proven experience with
home health admissions and OASIS documentation

• Ability to
think critically, prioritize quickly, and make confident decisions independently

• Strong communication skills with a focus on
patient education and care coordination

• Reliable transportation and comfort working in a
field-based environment

.
