Job Description

About exacare ai

exacare ai is a leading health tech company on a mission to build the AI operating system for post-acute care. Our platform turns messy, unstructured referral packets into clear clinical insights and next steps, so teams can make faster, safer placement decisions with less administrative burden. Today, exacare ai powers more than 2,500 facilities, and is growing rapidly.

We recently raised a $30M Series A led by Insight Partners, and are bringing world-class talent together to transform healthcare. If you like building, learning, and want to make a real impact, come join us!

About the Role

We are looking for a Staff Software Engineer who thrives in high-ownership environments and can drive complex product initiatives from concept to production. In this role, you will own user-facing features end-to-end—from understanding customer workflows and identifying the right product solution to designing, implementing, validating, deploying, and operating production-ready systems.

Success in this role requires systems-to-workflow thinking: the ability to connect software architecture, data models, APIs, integrations, and AI capabilities with the real-world experience of customers, implementation teams, and internal operators. You will balance long-term platform investments with pragmatic product delivery, ensuring that solutions are scalable, maintainable, and deliver meaningful customer value.

As a Staff Software Engineer, you will have a meaningful impact on the future of exacare's platform by shaping the technical direction across multiple product areas. You'll identify reusable patterns, abstractions, and frameworks that accelerate engineering velocity and improve product consistency. Working closely with product, design, clinical, and customer-facing teams, you'll solve ambiguous, high-impact problems, influence architectural decisions, and raise the engineering bar across the organization as we scale.

What You’ll Do

  • Own product features end-to-end: Take a feature from workflow problem through technical design, implementation, verification, production rollout, and operational readiness — without requiring constant senior oversight.
  • Bring systems-to-workflow thinking: Reason across software architecture, data models, integrations, and reusable product patterns — always grounding decisions in how the customer, operator, or implementation team experiences the result.
  • Build and ship user-facing product: Design and deliver reliable full-stack features that support complex healthcare workflows at scale, using TypeScript, PostgreSQL, and modern cloud-native tooling on AWS.
  • Drive quality and production readiness: Maintain a high quality bar across testing, observability, deployment safety, and incident response — proactively managing rollout risk and operational impact.
  • Create reusable patterns and frameworks: Identify opportunities to abstract common workflows, integrations, or AI-powered patterns across product surfaces — creating leverage for the broader engineering team.
  • Collaborate cross-functionally: Work closely with product, operations, platform, ML, and other engineering partners to translate real workflow problems into robust, maintainable technical solutions.

What You’ll Bring

  • Strong full-stack engineering fundamentals — proficiency with TypeScript, PostgreSQL, REST APIs, and modern backend architecture.
  • 7+ years of software engineering experience with a focus on product engineering, backend systems, or full-stack development.
  • Ability to own production-facing features end-to-end — from design to rollout, without requiring constant senior oversight.
  • Good product and workflow judgment — you think about the customer or operator experience, not just the technical implementation.
  • Comfort operating in ambiguity — you can scope, plan, and execute independently on high-impact, open-ended problems.
  • Clear communication with product, operations, platform, ML, and other engineering partners.

Nice to Have

  • Experience building applied AI features or working with LLM infrastructure.
  • Experience with healthcare workflows, care-setting variability, or regulated environments.
  • Experience with EHR integrations or health data standards.
  • Experience with operational tooling or onboarding/support-heavy product surfaces.
  • Experience building reusable patterns or frameworks that help teams move faster.
  • Bachelor's degree in Computer Science or a related technical field (or equivalent practical experience).

Benefits + Perks

  • Competitive salary and equity in a high-growth startup
  • Flexible PTO, take what you need
  • Medical, dental, and vision coverage
  • Great startup culture, including company off-sites
  • High-achieving team, including ex-Amazon, Google & Tesla engineers and alumni of Bain, BCG, Goldman Sachs, and more

An insight into our Core Values

Only the Best Belong Here

We are unapologetic about talent. This should be the best team you have ever been on. Protecting that standard is how we honor each other’s time, ambition, and craft.

We work even harder to keep our partners than we did to earn them initially

The work does not stop when a customer first onboards to our platform. It deepens over time. We partner with operators, listening and learning about real problems, and translate that into solutions that help them succeed in practice. We earn trust through consistent delivery.

The Patient is Downstream of Every Decision

At the end of the day, this is about the patient. We get there by deeply respecting post-acute operators and partners, because their work is the path our software travels to create better care.

Raise the Bar on Ownership

We grow because people here go beyond the minimum. We invest extra effort, care, and ownership into what we build.

The World is Moving Fast. We move Faster.

This is a race. We work hard, we move early, and we stay ahead of problems and competitors. If we slow down, someone else will pass us.

Radical Candor, Zero Politics.

We say what’s true, early, and we keep communication direct and clean so the team can move.

Bring Good Vibes and Win Together

We win as a team. We bring energy, support each other, and make the workplace somewhere people are excited to show up to.

If this sounds like you, we'd love to have a chat!
