​​The Staff Physician provides high-quality, evidence-based primary care in a 100% outpatient setting, with a strong focus on prevention, obesity, diabetes, and related cardiometabolic conditions. This role cares for a diverse patient population with significant chronic disease burden and works closely with a multidisciplinary care team to improve individual and population health outcomes.

​Reporting to the CMO or CMD, the Staff Physician will provide comprehensive, longitudinal care while partnering with primary care providers, behavioral health, physical therapy, health coaching, and other clinical team members to deliver coordinated, patient-centered care.​

This is an onsite position based in Lost Hills and Delano, CA. Relocation Assistance is available.

Essential Job Duties & Responsibilities:

  • Provide compassionate, high-quality, evidence-based primary care, including preventive care, chronic disease management, and evaluation and treatment of acute conditions
  • Diagnose, treat, and manage obesity and obesity-related conditions, including diabetes, hypertension, dyslipidemia, and other cardiometabolic diseases
  • Develop individualized treatment plans and partner with patients to support sustainable improvements in health, disease management, and overall well-being
  • Manage an empaneled patient population and coordinate care across the multidisciplinary clinical team to improve clinical outcomes and continuity of care
  • Collaborate closely with physicians, advanced practice providers, behavioral health providers, physical therapists, health coaches, medical assistants, and other members of the care team
  • Provide clinical guidance to care team members as appropriate to support safe, coordinated, and effective patient care
  • Participate in clinical quality improvement and population health initiatives focused on prevention, chronic disease management, patient engagement, and access to care
  • Review clinical and population health data to identify opportunities to improve patient outcomes and quality of care
  • Coordinate appropriate referrals, follow-up care, and specialty services to promote continuity and timely access to care
  • Support onsite, mobile, and community-based efforts designed to improve patient engagement, access, population health management, and clinical quality
  • Maintain accurate and timely medical documentation and adhere to applicable clinical, regulatory, and organizational standards
  • Provide care in a 100% outpatient practice, seeing an average of approximately 16 patients per day, with no evening or weekend call responsibilities

Qualifications

  • MD or DO degree from an accredited medical school
  • Board-certified in Family Medicine or Internal Medicine preferred
  • Current or eligible for unrestricted California medical licensure
  • Current DEA registration or eligibility to obtain registration
  • Current BLS certification or ability to obtain prior to start date
  • Experience providing outpatient primary care strongly preferred
  • Experience managing patients with obesity, diabetes, and other cardiometabolic conditions preferred

Knowledge, Skills & Abilities:

  • Strong clinical knowledge of comprehensive adult primary care, preventive medicine, and chronic disease management
  • Knowledge of evidence-based approaches to obesity, diabetes, and cardiometabolic disease prevention and management
  • Demonstrated commitment to improving health outcomes for populations experiencing significant chronic disease burden and barriers to care
  • Strong patient communication skills with the ability to engage patients in shared decision-making and long-term disease management
  • Ability to work effectively within an integrated, multidisciplinary care model
  • Written and verbal fluency in medical Spanish strongly preferred
  • Proficiency with electronic medical records and standard workplace technology
