We are looking for a compassionate and dedicated Staff Nurse (Homecare) who is passionate about providing quality patient care and able to provide consultative advice and home care services to patients and family members based on care needs.This role will also support families and care givers of patients and work with the rest of the multi-disciplinary team to provide symptom management, nursing care, psychological, emotional and spiritual care.

Responsibilities:

  • To uphold nursing standards for patients admitted to homecare as well as care for families and loved ones.

  • To work in collaboration with the Advanced Practice Nurse, NC/NM/NE and other nursing staff as appropriate to put in place clinical processes that are conducive to the maintenance and improvement of standards of nursing care in Home Care.

  • To ensure accuracy of all documentations related to patient care in the system.

  • To ensure holistic assessment of patients in DPH Home Care and their families and caregivers in order to formulate a core plan.

  • To be part of the nursing teaching faculty at DPH as required and to be part of the process of quality improvement through review of care processes, active participation and/or leadership in projects as required.

Job Requirements:

  • State Registered Nurse with minimum 5 years of nursing experience in acute care setting, and 3 years as a staff nurse in a palliative care setting.

  • Post-Basic training in Palliative Care Nursing is preferred.

  • Training in lean methodology, audit and research is preferred.

  • Ability to speak a second language or other local languages

  • Mature, dynamic, and self-motivated.

  • Possess excellent communication and interpersonal skills.

  • Self-directed with the ability to work with little supervision.

  • Comply with accepted professional standards and practices.

  • Possess satisfactory references from employers and / or professional peers.

  • Flexible and cooperative in fulfilling all obligations.
