The Senior Director of Home and Community Based Services is responsible for the overall leadership, strategic direction, and operational performance of the Home Health, Hospice, and Home Care programs which meets the highest standards defined by the industry and regulatory bodies, and in keeping with the appropriate desires of the residents and their families. This role ensures high-quality, compliant, and financially sustainable services across all lines of business, while supporting growth, staff engagement, and exceptional patient and family experiences. The Senior Director partners closely with clinical, financial, and operational leaders to align care delivery with organizational goals, regulatory requirements, and community needs. Collaborates closely with the home health, hospice and home care leaders to ensure seamless delivery of quality services. Serves as facility administrator at Dozoretz Hospice House of Hampton Roads. What You’ll Do In This Role

  • Provide day-to-day oversight of Home Health, Hospice, and Home Care operations.
  • Ensure seamless coordination across service lines to support patient transitions and continuum of care.
  • Establish and monitor key performance indicators (KPIs) including census, admissions, visit utilization, and productivity.
  • Drive operational efficiency while maintaining high standards of care and service delivery.
  • Develop and manage operating budgets for all programs.
  • Monitor financial performance including revenue, labor, productivity, and cost per visit.
  • Align staffing and operations with census and volume trends to protect margins with the respective business leader.
  • Collaborate with billing and finance teams to ensure clean claims, timely billing, and strong cash flow.
  • Ensure compliance with Medicare Conditions of Participation for Home Health and Hospice.
  • Maintain survey readiness and lead responses to audits or regulatory reviews.
  • Oversee policy and procedure development and implementation.
  • Identify opportunities for program growth and service expansion.
  • Partner with business development teams to strengthen referral relationships and market presence.
  • Monitor market trends and adjust strategies accordingly.
  • Lead, mentor, and develop interdisciplinary teams across all service lines.
  • Ensure strong leadership structure (clinical, operations, scheduling, intake).
  • Promote a culture aligned with organizational values (e.g., respect, passion, professionalism).
  • Support recruitment, retention, and engagement of staff.
  • Partner with executive leadership on strategic initiatives and performance reporting.
  • Collaborate with internal departments including IT, HR, finance, and compliance.
  • Serve as a key liaison to physicians, partners, and community stakeholders.
  • Participate in board-level reporting as needed.
  • Assures optimal utilization of available financial resources within the context of Westminster-Canterbury’s standards of care and values.
  • Maintain current knowledge of State and Federal regulations and ensure that operations and practices are in compliance with these various regulatory and licensure requirements at all times.
  • Manage Director of Home Health, Hospice, and Home Care.
  • Collaborate with other leadership team members to maximize synergies with Value Based programs, Evidence-Based Care, Process Excellence, Infection Control, Patient Safety, and Regulatory and Accreditation compliance.
  • Responsible for managing the Compliance of Home Health, Hospice, and Home Care, and oversees the annual Survey process.
  • All other duties as assigned.

Physical Requirements

  • The ability to climb or descend flights of stairs, standing or walking for extended periods.
  • Must be able to sit up to 8 hours a day; stand up to 4 hours a day; drive up to 8 hours a day.
  • Office environment with standard business equipment
  • Ability to work extended hours during partner onboarding/offboarding, audits, and regulatory deadlines.
  • May require local travel between office locations, patient care settings, and partner organizations.
  • Participation in on-call rotation as needed for administrative oversight.
  • Physical demands: Prolonged sitting, computer work, and presentation activities
  • Ability to work in office and field environment
  • Climb at least 2 flights of stairs

The Experience You’ll Bring

Required

  • Bachelor’s degree in Nursing, Healthcare Administration, or related field required.
  • Active and Unencumbered Registered Nurse (RN) license issued by the state of Virginia or compact state.
  • Minimum of 5–10 years of leadership experience in Medicare-certified home health or hospice, including at least 5 years in a supervisory or administrator-level role.
  • Strong knowledge of Medicare Conditions of Participation.
  • Deep knowledge of Medicare reimbursement models, including PDGM (Home Health) and Hospice per diem with levels of care (Routine, Continuous, GIP, Respite).
  • Experience with OASIS, HOPE, survey processes, and regulatory compliance.
  • Proven success in managing operations, financial performance, and team leadership.
  • Proficient with computers and tablets, as well as electronic medical record (EMR) and basic computer applications.
  • Experience with regulatory compliance, surveys, and accreditation (CHAP, ACHC, or Joint Commission).
  • Valid state driver’s license.
  • Motor Vehicle Driving Record satisfactory to Westminster-Canterbury and its insurance providers.
  • Proof of motor vehicle insurance and reliable transportation.
  • Flu-Vaccinated

Preferred

  • Multi-site or multi-service line leadership experience (home health, hospice, and personal care/home care).
  • Demonstrated success managing P&L responsibility, including budgeting, forecasting, and margin improvement.
  • Experience leading organizations through regulatory surveys and accreditation (CHAP, ACHC, Joint Commission) with strong outcomes.
  • Proven ability to drive census growth, referral development, and market expansion.
  • Experience implementing or optimizing EMR systems (e.g., MatrixCare) and leveraging data for operational and clinical decision-making.
  • Strong background in QAPI, compliance, and risk management, including policy development and audit readiness.
  • Experience leading organizational change, including staffing model redesign (e.g., salaried clinicians, productivity models).
  • Prior experience in a non-profit or mission-driven healthcare organization.
  • Experience working with boards, executive leadership, and external partners (e.g., hospitals, CCRCs, physician groups).
  • Ability to balance clinical quality with financial performance in a value-based care environment.

  • Compensation Range: $133,399.00- $170,083.00 annually (based on relevant experience)
