Job Description

Full Time

The Case Manager responsibilities include: developing and implementing service plans for participants, conducting vocational and other assessments for participants in accordance with the participants specific program requirements and completing case notes, and other related documents.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
The requirements listed below are representative of the essential duties required of the Case Manager position.

• Develop program participant’s service plan and goals with input from the program participant, relevant Goodwill staff, work site managers, and appropriate family/home supports. Service plans and goals should be completed using the required assessment, and service plan documents. Service plans should be completed and submitted in accordance with the program specific requirements.
• Provides on-going case management services to program participants.
◦ Consults with and coaches participants to support them as they work on achieving their specific goals, as outlined in the annual service plan documents.
◦ Designs and implements targeted interventions and supports as needed to assist program participants in achieving their goals.
◦ Performs regular appropriate and required assessments with participants.
◦ Conducts regular job site visits for participants working in the community.
◦ Provides vocational counseling to program participants and acts as referral source for additional counseling services as needed.
◦ Coordinates additional services as needed e.g., transportation
• Participates in intake, orientation, and referral process for the program participants.
◦ Participates in (and may lead) program tours for referred individuals, families, and staff, if appropriate.
◦ Provides input on potential participant eligibility into a Goodwill program.

• Tracks and reports relevant data, including case notes in ETO database.
• Attends and participates in various organizational and departmental meetings.
◦ P&S staff meetings and trainings when appropriate.
◦ Worksite general staff meetings.
◦ Attendance at other meetings as requested (i.e., committees and special projects).
• Attends training opportunities that enhance job related skills.
• Maintains practical and up-to-date case manager knowledge and best practices.
• Performs duties in accordance with Goodwill’s standard policies and procedures.
• Other duties as assigned.

LEVEL OF RESPONSIBILITY
• Employee may work independently, but work is often reviewed by immediate supervisor.
• Employee exercises discretion within area of responsibility.
• Employee advises others within the organization on matters within area of expertise.

SUPERVISORY RESPONSIBILITIES
• Does not have any supervisory responsibilities.

QUALIFICATION REQUIREMENTS:
• Bachelor’s degree (preferred) or Associate’s degree and two years of related experience.
• One (1) year of work experience in addition to above.
• Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Skill to write reports, business correspondence and procedure manuals.
• Capability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Aptitude to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
• Reliable transportation required. (May require valid driver’s license).
• Proficiency in Microsoft Office, particularly Word, Excel, PowerPoint, and Teams.
• Satisfactory results of all required background checks.

WORKING CONDITIONS AND PHYSICAL EFFORT
The work environment characteristics described here are representative of those a Case Manager encounters while performing the essential functions of this job.

Physical Demands/Efforts
• Must be able to sit for prolonged periods of time working on a computer.
• Work performed is typically sedentary with occasional periods of walking, standing, and reaching.
• May require stooping, kneeling, crouching, reaching, and crawling.
• Utilizes finger dexterity to perform computer operations, handle or feel objects, tools, or controls.
• May be required to lift or move up to twenty (20) pounds at times.

Mental & Visual Demands
• Considerable mental concentration to effectively execute complex tasks.
• Work occasionally requires visual effort for extended periods of time.
• Specific vision abilities required by this job include depth perception, peripheral vision, the ability to distinguish basic colors and the ability to adjust vision to bring objects into focus.

Work Environment and Hazards
• Working environment varies between an office setting where the noise level is usually moderate and a classroom or warehouse where the noise level is typically moderate to high.
• Absence of hazards.
