Job Description

This is a full-time professional position. This position is part of the Hospice interdisciplinary team and is responsible to develop and implement a care plan to provide spiritual care and support to the patient and family. The schedule for this position is Monday-Friday 8:30am-5:00pm; weekends as needed; 1 major or 2 minor holidays per year

Responsibilities what you do

  • Provide immediate and long-term assessment, and documentation, of the spiritual situation and needs of patients and families assigned to your care area.
  • Collaborate with the patient, the patient’s family, facility staff and the Hospice Interdisciplinary Group (IDG) to develop, spiritually & culturally sensitive, spiritual care plan components.
  • Provide and document spiritual care and counseling interventions to patients and families, to include the full range of sacraments and rituals of the patient and/or family’s spiritual/religious community.
  • Provide, and document provision of, appropriate and relevant end of life information and education to facility staff, patients and their families in compliance with current Centers for Medicare and Medicaid Services (CMS) Conditions of Participation (CoPs).
  • For persons of other faith perspectives who wish ministry from within their faith, identify and make referrals to appropriate and available clergy or religious leaders in the community.
  • Provide spiritual support to hospice staff and partnering facility staff in your assigned areas through individual counseling, educational in-services and memorial services.
  • Participate in the identification of clinical or operational performance improvement opportunities and in performance improvement activities.
  • Responsible for maintaining current knowledge of applicable spiritual care counselor practices.
  • Responsible for professionally managing time, people and technology.
  • Actively participates in the orientation and training of new employees and students in all disciplines.
  • Works collaboratively with IDG and other Transitions LifeCare (TL) departments in achieving patient care goals.
  • Maintains patient confidentiality at all times.
  • Perform other duties as assigned by supervisor.

Requirements what you need

  • MDiv, Master of Pastoral Care or Master of Theological Studies from accredited university
  • Completion of Clinical Pastoral Education residency preferred or equivalent experience
  • Minimum one-year experience in health-related field.
  • Hospice experience preferred.
  • Able to use computer and learn new software programs
  • Must have reliable transportation.
  • Must be able to travel to, enter and function in the homes/facility of any patient/family in the service area to assess and provide care to all patients.
  • May be required to lift and/or carry items up to 25 pounds.
  • Must have sensory abilities to complete physical assessment, communicate with patient/family care providers and physician and to complete the reporting and follow-up documentation responsibilities of the job.
  • Work hours and days including weekend and holiday commitments as outlined in Employee Information Agreement.
