Chandler Creek Post Acute is a 133-bed skilled nursing facility located in Greer, South Carolina. We take pride in our small-town feel, strong community connections, and commitment to providing compassionate, high-quality care.

We are seeking a dedicated Speech-Language Pathologist (SLP) to join our full-time rehab team. In this role, you’ll have the opportunity to make a meaningful impact by helping residents improve communication, cognition, and swallowing function.

As a Speech-Language Pathologist (SLP), you will evaluate, treat, and support residents with speech, language, cognitive, and swallowing disorders. You’ll collaborate closely with physicians, nursing, and therapy teams to deliver individualized, patient-centered care.

Key Responsibilities

  • Perform evaluations within 24 hours (or 1 business day) of physician referral
  • Develop and implement individualized treatment plans with physician approval
  • Provide therapy services in accordance with treatment plans
  • Monitor patient progress and adjust care as needed
  • Maintain accurate clinical documentation (evaluations, progress notes, discharge planning)
  • Participate in care plan meetings, utilization review, and interdisciplinary conferences
  • Educate residents, families, and staff on therapy programs and techniques
  • Supervise SLPAs and Clinical Fellows (CFY) as applicable
  • Conduct staff training and in-service education
  • Ensure compliance with all regulatory and facility standards

Qualifications

  • Master’s or Doctoral degree in Speech-Language Pathology
  • Active, unencumbered SLP license in South Carolina (required)
  • CCC-SLP preferred
  • Experience in a skilled nursing or rehab setting preferred
  • Knowledge of speech, language, cognitive, and swallowing disorders
  • Experience with PointClickCare (PCC) and Casamba preferred
  • Strong clinical, communication, and documentation skills
  • Ability to work both independently and as part of a team
