Victorian Post Acute is a 90-bed, rehab-focused skilled nursing facility dedicated to providing exceptional care for both short-term rehabilitation and long-term care residents. We are seeking a compassionate and motivated PRN Speech-Language Pathologist (SLP) to join our collaborative in-house therapy team.

Key Responsibilities
Evaluate residents and develop individualized speech-language pathology treatment plans.
Provide skilled therapy services for speech, language, cognitive-communication, voice, and swallowing disorders.
Perform clinical swallowing evaluations and implement appropriate dysphagia management strategies.
Collaborate with physicians, nursing staff, dietitians, rehabilitation professionals, residents, and families to achieve optimal outcomes.
Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
Participate in interdisciplinary care conferences and discharge planning.
Educate residents, caregivers, and staff on communication strategies, swallowing precautions, and home programs.
Ensure compliance with all federal, state, and facility documentation and regulatory requirements.
Qualifications
Current California Speech-Language Pathologist (SLP) license in good standing.
Certificate of Clinical Competence (CCC-SLP) preferred or eligibility as required by state regulations.
Excellent communication and interdisciplinary collaboration skills.
