McClure Post Acute is seeking a compassionate, knowledgeable, and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis. If you're passionate about helping residents improve their communication, cognition, and swallowing abilities while working in a collaborative post-acute environment, we'd love to hear from you.
As a valued member of our interdisciplinary team, you will evaluate, develop, and implement individualized treatment plans that promote functional independence and improve quality of life for our residents.
What You'll Do
Evaluate residents to determine speech, language, cognitive, voice, and swallowing needs.
Develop and implement individualized treatment plans based on physician orders and resident goals.
Provide evidence-based therapy to improve communication, cognition, swallowing safety, and overall functional outcomes.
Monitor resident progress and modify treatment plans as appropriate.
Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and family members to ensure coordinated care.
Maintain timely, accurate, and compliant clinical documentation.
Participate in interdisciplinary care plan meetings, rehabilitation conferences, and discharge planning.
Educate residents, families, and caregivers on therapy techniques, home programs, and aspiration precautions.
Assist with staff education and provide clinical in-services as needed.
Supervise Clinical Fellows (CFY) and Speech-Language Pathology students when applicable.
Maintain compliance with all facility, state, federal, and professional practice standards.
Qualifications
Master's Degree in Speech-Language Pathology from an accredited program.
Current, unrestricted California Speech-Language Pathologist (SLP) license.
Certificate of Clinical Competence (CCC-SLP) preferred.
Clinical Fellows (CFY) are encouraged to apply if eligible for supervision.
Skilled nursing or post-acute care experience is preferred but not required.
Excellent communication, documentation, and interpersonal skills.
Strong commitment to providing compassionate, resident-centered care.
