What You'll Do


  • Perform comprehensive speech, language, cognitive, and swallowing evaluations in patients' homes.

  • Develop individualized treatment plans focused on meaningful functional outcomes.

  • Provide evidence-based interventions for communication, cognition, and dysphagia.

  • Educate patients, caregivers, and family members to promote long-term success.

  • Collaborate closely with physicians, nurses, therapists, and the interdisciplinary care team.

  • Complete timely and accurate documentation using our electronic medical record.


What We're Looking For


  • Active Minnesota
    Speech-Language Pathologist (SLP)

    license.

  • Passion for helping patients regain independence and confidence.

  • Excellent communication and clinical reasoning skills.

  • Home health experience is welcomed but not required.

  • Valid driver's license and reliable transportation.
