St. Francis Healthcare Center is seeking a compassionate and motivated Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis. As an SLP at St. Francis Healthcare Center, you'll collaborate with an interdisciplinary team dedicated to delivering exceptional patient-centered care and helping residents achieve their highest level of independence.
Evaluate residents with speech, language, cognitive, voice, and swallowing disorders.
Develop and implement individualized treatment plans based on patient needs and physician orders.
Provide skilled therapy services to improve communication, cognition, and swallowing function.
Monitor patient progress and adjust treatment plans as appropriate.
Educate residents, families, and caregivers on treatment goals and home strategies.
Maintain accurate, timely, and compliant documentation.
Collaborate with physicians, nursing staff, therapists, and other healthcare professionals to coordinate patient care.
Ensure compliance with all federal, state, and facility regulations.
Current Speech-Language Pathologist license in the State of California.
Certificate of Clinical Competence (CCC-SLP) preferred; strong CF candidates may be considered where applicable.
Experience in a skilled nursing facility or post-acute rehabilitation setting is preferred.
Knowledge of dysphagia assessment and treatment.
Excellent communication, organization, and interpersonal skills.
Ability to work independently while contributing to a collaborative team environment.
