
Assists in providing services to patients, their families, or significant others who are in need of help with social, economic, or environmental problems resulting from, or related to their behavioral health diagnosis. Using creative communication strategies, the Social Worker / Discharge Planner will assist IDBH patients to anticipate, identify and overcome barriers within their journey through the healthcare system. The Social Worker / Discharge Planner works closely with the members of the multidisciplinary behavioral health team, including patients, families, physicians, nursing staff, and many other disciplines in the Iredell Health System involved in their care. Assists the patient in understanding their diagnosis, coordinating their treatment plans, seeking assistance from various appropriate social services and assisting the patient in communicating their treatment plan with their family and caregivers, documenting all interactions and outcomes. Partners with a network of community agencies to meet identified psychosocial needs of the patient. Has full access to patient health information.

When Iredell Memorial Hospital opened in 1954, we were a 100-bed hospital with a mission focused on the well-being of the community we serve. The name, Iredell Memorial Hospital, was selected by a board of trustees as a memorial to all members of armed forces from Iredell County who gave their lives in defense of the United States.
Although we have grown to 391 licensed beds to meet the needs of the community, the buildings and grounds are still owned by the citizens of Iredell County and all revenue is reinvested in medical technology and staff. Because we are a nonprofit hospital, our focus is on promoting health and wellness in our community, not on the bottom line.

As numerous freestanding services were added to meet the needs of the community, we became Iredell Health System. Today we employ almost 2,000 people, making us the third-largest employer in Iredell County. There are approximately 365 physicians representing a variety of specialties.
