Sunterra Springs - Dardenne Prairie is seeking an energetic and dynamic individual as a Full-time Social Services/DC Planner in our skilled nursing facility. We are looking for someone who is experienced in healthcare to join our amazing team and provide social services and DC planning for our patients and residents under our care.

Responsibilities include:

  • Provides social services to meet the needs of the residents and family
  • Demonstrate ability to engage interdisciplinary team, staff, patient, family members and understands and can provide the emotional resource necessary for end of life care as needed
  • Attends all IDT and mandatory meetings and communicates effectively with interdisciplinary team members
  • Completes all required documentation in compliance with rules and regulations including documentation of communication skills and activities with the patient such as assessment of emotional factors, counseling for long-range planning and decision-making, community resource planning, short-term therapy, etc.
  • Provide timely and effective discharge planning
  • Others duties as assigned

Position Requirements:

  • Excellent punctuality and attendance
  • Nursing home experience is a must
  • Experience in Discharge Planning
  • Ability to multitask and meet deadlines
  • Must possess solid knowledge of a clinical environment
  • Patience and professionalism

License/Certification preference:

  • Resident Advocate (with experience)
  • Social Worker
