Summary

First point of contact to screen and refer clients to appropriate services. Provides education on available services, referral to service connection, and development and monitoring of interdisciplinary treatment plans. May provide community outreach, education, and referrals to community members. Prepares documentation for commitment and recommitment hearings, including scheduling witnesses and communicating with probate.

Examples of duties

First point of contact for clients, families, referral sources, community agents, third party payers and others regarding referrals for services as needed; provides accurate and timely documentation of client status, progress and needs.

Demonstrates familiarity with client and other rights. Maintains the confidential nature of client and business information.

Receives direct referrals for services and conducts screening for service need, including facilitating court referrals accordingly. Ensures clients and relatives understand the nature of three-party petitions, probable cause hearings, commitment hearings, and recommitment hearings.

Assists with accepting referrals for funding of substance use disorder residential treatment and detox and assists with screening for appropriate funding source and referral.

Participates on interdisciplinary treatment teams and provides connection to appropriate services.

Provides referral to services for consumers requiring inpatient hospitalization who have not yet been assigned a case manager.

Compiles, maintains, and analyzes data related to consumer and community needs.

Demonstrates an understanding and referral of dual diagnosis/co-occurring services.

Attends and positively contributes to staff meetings and department performance overall.

Attends in-service and outside agency training sessions as appropriate.

Complies with the Human Services and/or Community Treatment Center policies.

Qualifications

Education and Experience:

Bachelor’s Degree in Social Work, Psychology, Counseling or closely related field.

At minimum two years of experience in SUD/mental health field, with experience in crisis intervention preferred.

Licenses and Certifications:

Valid Wisconsin Driver’s License.

Supplemental information

Knowledge of applicable laws (Chapter 45, 51 & 55) and related court procedure.

Knowledge of community human services resources and utilization.

Ability to screen for client needs and complete appropriate referrals as appropriate.

Ability to maintain effective working relationships with other staff and the public.

Knowledge of and ability to utilize a computer and the required software.

Ability to work independently while exercising sound judgment, particularly in high-pressure or challenging situations.

Ability to establish and maintain effective working relationships with other staff and the public.

Ability to communicate effectively both orally and in writing.

Ability to accept and constructively use criticism.

Ability to maintain confidentiality.

Ability to maintain records and reports timely and accurately.

Ability to work the required hours of the position.

PHYSICAL DEMANDS:

Lifting 20 pounds maximum with frequent lifting and/or carrying of objects weight up to 10 pounds.

Intermittent standing, walking and sitting; occasional driving.

Using hand(s)/feet for repetitive single grasping, fine manipulation, pushing and pulling, and operating controls.

Occasional bending, twisting, squatting, climbing, reaching and grappling.

Communicating orally in a clear manner.

Distinguishing sounds at various frequencies and volumes.

Distinguishing people or objects at varied distances under a variety of light conditions.
