Hours: Temporary full time for up to 4 months

What you’ll be doing:

  • Conduct a comprehensive assessment for the client relevant to biopsychosocial history, diagnosis and ability to make effective use of treatment
  • Integrate the biopsychosocial history into the treatment plan
  • Develop and evaluate treatment plans in collaboration with the clinical team, community supports and families.
  • Facilitate group counselling sessions
  • Provide individual counselling in support of patient’s specific needs and goals
  • Ensure continuity of care is maintained through the transfer of information
  • Work with the clinical team to initiate community treatment orders
  • Establish relationships with key community supports
  • Discharge planning
  • Prepare referrals to appropriate community services
  • Document in a timely manner
  • Collaborate with the multidisciplinary team

What we’re looking for:

  • Master’s Degree in Social Work
  • Current registration with the Ontario College of Social Workers
  • Training and educated in CBT and/or DBT modalities, and Mindfulness
  • Experience working with individuals with complex mental health and addiction issues, and trauma
  • Experience in program development and delivery
  • Experience with family therapy and/or motivational interviewing preferred
  • Ability to work independently and within an interdisciplinary team
  • Good communication and organizational skills
  • Effective computer skills and record keeping
