Job Description

Roles & Responsibilities:

CASEWORK AND COUNSELLING

  • Manage 25-30 C&C active cases (mainly low-moderate level of complexity) per quarter.
  • Conduct assessment, planning and intervention for cases by taking into account the integration of a range of information, including practice guides and organisational frameworks.
  • Conduct case conferences with partners.
  • Participate in networking and case management meetings.
  • Document and evaluate case outcomes.
  • Develop and review appropriate intervention strategies.
  • Ensure proper documentation of cases.
  • Ensure reports on cases are submitted in a timely manner.

INFORMATION AND TRIAGE

  • Manage case referrals.
  • Part of the Intake & Triage Team to conduct intake assessments for referred clients based on the practice standards of the Code of Social Work Practice and Practice Guide.
  • Refer clients to appropriate agencies for further intervention.

COMMUNITY WORK

  • Network with other organizations and stakeholders.
  • Participate in implementing and evaluating of community outreach / initiatives.

GROUP WORK

  • Be part of the programme team for Group Work conducted at the FSC.

REQUIREMENTS

· Degree or Graduate Diploma in Social Work or related field such as Psychology, and other relevant fields in Social Science

· Previous experiences via internships or placements in related areas of work will be considered

· Understanding of social work values and ethics

· Understanding of work with vulnerable populations i.e. low-income families
