Job Description

Devises/implements a social work treatment/continuing care plan involving the patient, family/significant other and the interdisciplinary team and documents on the patient’s medical record in accordance to the departmental policy.

Responsibilities

- Establish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employee.

- Contributes to and participates in the Performance/Quality Improvement activities of the assigned department. Contribution and participation includes data collection, analysis, implementation of and compliance with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the BronxCare Health System (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service.

- Completes Social Work Psychosocial Assessments for all patients referred or in need of social services in accordance with department’s high risk criteria policy.

- Demonstrates competency in identifying behavioral, cognitive, and/or mental health disorders and in recognizing the underlying factors for such disorders.

- Provides supportive counseling, crisis intervention, and grief counseling to patients and their families/significant others as appropriate.

- Devises/implements a social work treatment/continuing care plan involving the patient, family/significant other and the interdisciplinary team and documents on the patient’s medical record in accordance to the departmental policy.

- Demonstrates competency in effectively utilizing appropriate computer applications and programs within the scope of professional role and responsibilities.

- Provides direct/task supervision to social work students when designated.

- Identifies, evaluates and designs proper safety plans for victims of crimes/abuse, i.e.: child abuse, elder abuse, domestic violence, rape and sexual assault.

- Responds and participates in external and internal disasters when indicated/assigned by supervisor.

- Assists/participates in presentations, training and in-services as required.

- Interviews patients and family members to obtain information about home environment, family relationships and history, and the patient's strengths/weaknesses.

- Maintains and completes statistics of case activities as outlined in the departmental policy.

- Participates as a team member, on behalf of patients, in conferences with hospital personnel and community agencies.

- Maintains confidentiality of patient and hospital records and uses discretion when discussing patient and hospital matters.

- Maintains active and supportive communication with the patient and others concerning progress, problems, home programs, or other issues related to the patient's recovery.

- Presents appropriate appearance in accordance with department policy.

- Conforms to established time and leave regulations and policies.

- Performs other duties assigned by supervisor as needed.

Qualifications

- 0-3 years of experience in discharge planning, clinical and social welfare service. Knowledgeable of CMS and The Joint Commission regulations; Managed Care, NYSDOH and OMH guidelines; Milliman Care Guidelines and community resources.

- LMSW/LCSW

- MSW in Social Work

- Microsoft Office

- Knowledge Base Charting (KBC, Care Management (Allscripts)

- Internet Navigation, Outlook

- Knowledge of social work techniques in working with individuals, groups and families. Able to understand the impact of disease process on psychosocial functioning.

- Knowledge of Substance Use/Abuse: behaviors, interventions and resources.

- Knowledge of Child Abuse, Domestic Violence, Sexual Assault/Rape, and Elder Abuse: intervention techniques and resources.
