Job Category: Health CareProvidence Care is seeking a compassionate and experienced PRN Social Worker to support hospice patients and their families.
In this role, you will provide emotional support, resource coordination, and guidance through end-of-life care; working closely with an interdisciplinary team to ensure patients receive care with dignity and respect.
What You Will Do
- Complete psychosocial assessments for patients and families
- Provide counseling and emotional support
- Assist with care planning alongside the clinical team
- Connect families with community resources and services
- Support bereavement needs as appropriate
- Educate patients and families on available services and options
- Maintain accurate and timely documentation

QualificationsRequirements
- Bachelor’s or Master’s Degree in Social Work (CSWE accredited)
- Active South Carolina license (LMSW, or LISW-CP)
- Minimum of 1 year experience in a healthcare setting
- Hospice or end-of-life experience preferred
- Strong communication and interpersonal skills
- Ability to work independently and manage time effectively
