This position will provide direct clinical services to patients, and those involved in their care, who are facing serious illness, through consultative visits and telephonic outreach that facilitates advance care planning discussions and offers information/education regarding their illness trajectory and treatment options. Interventions will be delivered via telehealth to patients in a variety of care settings, including sub-acute rehab facilities and the home.
Key Responsibilities:
Provide information and education regarding illness progression and treatment options in collaboration with patient medical providers.
Support patients, and whomever they define as family, articulate health care goals, and provide information and referrals for emotional/spiritual resources as needed.
Advocate for goal-concordant treatment and care for patients and facilitate advance care planning.
Connect patients and families with hospice or other palliative care programs (if appropriate).
Manage, organize, and update relevant data using database applications.
Communicate effectively and efficiently with patients, caregivers, and interdisciplinary care team members.
Participate in Quality Improvement projects.
Work Location:
Remote – this role is eligible to work remotely, but the employee must be in the United States.

Qualifications:
Minimum Qualifications:
Principal Level:
A bachelor’s degree in health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
Three (3) years of relevant professional healthcare experience.
Possess a valid licensed Clinical Social Worker (LCSW) certification in the State of Colorado.
A combination of education and related technical/paraprofessional experience may be substituted for the bachelor’s degree on a year for year basis.
Applicants must meet minimum qualifications at the time of hire.
Preferred Qualifications:
A master’s degree in health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
One (1) or more years of palliative care experience.

Anticipated Pay Range:
This is an hourly position at $0.00. This position will be paid directly through the Holding Group.
The above salary range ( or hiring range) represents the University’s good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation, depending on the level.
