The Palliative Care Social Work Care Manager I serves as a key member of the ambulatory palliative care team, providing clinical social work services to patients living with serious, complex, or life-limiting illness and their families. The Social Work Care Manager utilizes clinical expertise to perform comprehensive psychosocial assessments, develop and implement care plans in collaboration with the appropriate care team, and assess crisis situations to provide clinical counseling, diagnosis, brief therapeutic interventions, and necessary resources or referrals.

This role also includes providing individual and family treatment as indicated, supporting patients and families with goals of care, advance care planning, caregiver needs, grief and loss, adjustment to serious illness, and other psychosocial concerns that may affect quality of life.

The position works collaboratively with patients, their support persons, healthcare providers, insurers, community resources, and all other involved parties as part of an interdisciplinary approach to palliative care.

Essential Functions

• Psychosocial Assessment and Screening: Evaluates patients and families for psychosocial, emotional, behavioral health, caregiver, substance use, social determinant of health, transition planning, and serious illness needs.
• Palliative Care, Therapeutic, and Crisis Support: Provides clinical social work services, brief individual and family therapy, supportive counseling, motivational interviewing, trauma-informed interventions, and crisis support related to serious or life-limiting illness, adjustment, coping, behavioral health, grief, loss, and life transitions.
• Care Planning and Coordination: Develops, implements, and monitors individualized care plans; addresses barriers to care; and coordinates medical, behavioral health, social, and community resources to help patients and families navigate serious illness and the healthcare system.
• Goals of Care and Advance Care Planning: Facilitates conversations regarding patient and family goals, values, preferences, and priorities and collaborates with the interdisciplinary team to support advance care planning and informed, patient-centered decision-making.
• Caregiver and Family Support: Assesses caregiver and family needs and provides education, counseling, resources, and support related to caregiving responsibilities, stress, anticipatory grief, loss, and changing family dynamics.
• Clinical Identification and Referrals: Identifies mental, emotional, and behavioral disorders when clinically indicated and connects patients and families with appropriate behavioral health, medical, social, and community services.
• Education, Advocacy, and Transitions of Care: Educates patients and families, advocates for patient rights and access to care, supports informed healthcare decisions, and assists with safe transitions to community-based care.
• Interdisciplinary Collaboration, Quality, and Compliance: Collaborates with healthcare teams, insurers, and community providers as an integral member of the palliative care team; participates in improvement initiatives; tracks key metrics; and follows applicable policies and standards of care.

Minimum Qualifications

• Master of Social Work (MSW) from an accredited institution (degree verification required).
• Current state licensure, as applicable, is obtained prior to or upon completion of required supervision hours.
• Basic computer proficiency, including familiarity with word processing and spreadsheet software.
• Strong written and verbal communication skills.
• Demonstrated ability to apply critical thinking skills.
• Caregivers whose duties require them to conduct home or community visits must maintain current BLS certification, have a current driver’s license, current auto insurance, an acceptable driving record and reliable transportation.

Preferred Qualifications

• Experience providing clinical social work services to patients and families coping with serious or life-limiting illness in palliative care, hospice, oncology, geriatrics, complex care, or a related setting.
• Experience supporting goals of care and advance care planning conversations.
• Experience providing counseling and psychosocial support related to serious illness, caregiver stress, grief, loss, and significant life transitions.
• Experience working as part of an interdisciplinary healthcare team.
• Experience in clinical care management, social work, or working with third-party payers, with a demonstrated understanding of care management principles and practices.
• Knowledge of community resources, healthcare systems, and social determinants of health.
• Additional education, training, or certification in case management, palliative care, hospice, serious illness care, grief and loss, or clinical social work.
• Ability to work independently, demonstrate self-motivation, maintain a positive attitude, and adapt to a rapidly changing environment.

Physical Requirements

• Ongoing need for employee to see and read information, documents, monitors, identify equipment and supplies, and be able to assess customer needs.
• Frequent interactions with providers, colleagues, customers, patients/clients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
• Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.
• May have the same physical requirements as those of clinical or patient care jobs, when the leader takes clinical shifts.
• For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$39.16 - $60.42
