Summary

Under supervision, performs a variety of case management services for the Perinatal Health Partners program.

Examples of duties

• Promote population health through illness, injury and disability initiatives.
• Deliver home visits for perinatal patients and infants; provide care coordination and referrals.
• Help staff contact patients and process initial referrals.
• Coordinate services with healthcare providers, agencies, and community organizations.
• Build and maintain community partnerships; represent program at outreach events.
• Maintain accurate patient records, service documentation, and treatment outcomes.
• Assist with inventory management and distributions.
• Perform clinical assessments of the behavioral health needs of patients by performing behavioral health screenings with the Edinburgh Postnatal Depression Scale.
• Refers client or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as transportation, financial assistance, legal aid, housing, job placement or education.
• Successfully completes training to become a Child Passenger Safety Technician and provides education to families receiving car seats.
• Supports multiple counties: Bleckley, Dodge, Johnson, Laurens, Montgomery, Pulaski, Telfair, Treutlen, Wheeler, and Wilcox.
• Assist with other program duties as assigned.
• Mileage is reimbursed.

Qualifications

Bachelor’s degree in a related field from an accredited college or university and two (2) years of experience in social service delivery.
