Job Description

Job Title: Social Worker - Neighbourhood Team
Location: London Borough of Lewisham
Pay Rate: £32 per hour

Liquid Personnel is looking for a skilled and compassionate Senior Social Worker to join its client's Mental Health Team based in London Borough of Lewisham.
This role involves leading on complex cases, supporting individuals with enduring mental health needs, and working collaboratively with health and social care professionals to promote recovery and independence.

What will your responsibilities be?

In this role, you will,

  • Ensure service users and their carers receive timely and proportionate care management assessments that identify the outcomes they wish to achieve.
  • Promote service user independence and recovery by encouraging Enablement and connecting individuals to community-based services that enhance personal wellbeing.
  • Collaborate within multi-agency teams to deliver seamless health and social care services.
  • Partner with Support Planning Officers to involve service users and carers in individualized, creative support planning, promoting choice and control through personal budgets, with Direct Payments as the default.
  • Work alongside Community Development Staff to facilitate access to community-based services.
  • Adhere to risk management and adult safeguarding procedures in social work practice, and remain vigilant to potential children’s safeguarding concerns, liaising with Children’s Social Care Services as appropriate.
  • Conduct assessments and service user reviews to a high standard and in a timely manner, ensuring they reflect the needs and desired outcomes of both service users and carers.

Qualifications & Experience:

To be successful in this role you must have,

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
