  • 0.4 FTE, Max Term until Mid July 2027 (Flexible days)
  • Based in Campbelltown on the lands of the Dharawal People
  • Flexible working arrangements are available, with community-based outreach work across the South Western Sydney catchment required

About the program

My Care Partners is a primary care-led program supporting people living with chronic and complex health conditions to stay well, connected, and supported in the community. The program brings together health and psychosocial expertise to address the full range of factors that influence wellbeing, with a strong focus on equity, access, and person-centred care.

Your role, your impact

As a Social Worker in the My Care Partners Program, you will support people navigating the psychosocial challenges that often sit alongside chronic and complex health conditions. Your work will help reduce barriers to care, strengthen individual resilience, and support people to feel safer, more connected, and more empowered in their daily lives.

Working closely with Registered Nurse Care Coordinators and primary care teams, you will contribute to integrated, holistic care that responds to both health and social needs.

You will:

  • Deliver psychosocial support and care coordination for people with chronic and complex health conditions in the community
  • Conduct holistic psychosocial and risk assessments to identify factors impacting health, safety and wellbeing
  • Develop and implement individualised care plans in partnership with clients, families and multidisciplinary teams
  • Provide short to medium-term therapeutic support, including counselling for anxiety, depression, grief, stress, family violence and adjustment-related concerns
  • Support goal setting and self-management, strengthening people’s capacity to navigate health and community services
  • Assist with care navigation, including support with housing, income support, NDIS and other service systems
  • Coordinate access to supplementary services funding to reduce financial barriers and promote equitable access to care
  • Participate in case conferencing and multidisciplinary meetings to enhance continuity and quality of care
  • Maintain accurate clinical documentation and deliver services in line with professional, ethical and legislative standards
  • Contribute to quality improvement initiatives and positive team culture

Who we’re looking for

You are a qualified Social Worker who brings empathy, strong professional boundaries and a commitment to person-centred, trauma-informed practice. You are comfortable working with complexity and value collaboration as much as autonomy.

You will bring:

  • Experience supporting people with complex psychosocial needs, including mental health and/or substance use
  • Strong assessment, counselling and care coordination skills
  • A trauma-informed, culturally responsive and inclusive approach
  • Confidence working within multidisciplinary health teams and primary care settings
  • Highly developed communication, organisation and time‑management skills
  • Sound judgement, emotional intelligence and the ability to work within scope of practice

Mandatory requirements

  • Bachelor’s degree in Social Work, providing eligibility for AASW membership
  • Minimum two years’ experience working as a qualified Social Worker
  • Current driver’s licence and willingness to travel for community and home-based work
  • Criminal History Check, Employee Working with Children Check and National Worker Screening Check
