Job Description

This position provides psychosocial assessments and re-assessments, family consultation, supportive counseling, support groups, information and referral, and education for the target population. Maintain a caseload of clients by monitoring individual plans of client self-care, coordination of services and ensuring follow-up on grant funded services for clients and their care recipients.

Provides support to and builds relationships with high risk, homeless and mental health patients who are frequent emergency department admissions and /or who discharged from the hospital to ensure a successful transition from ED/ hospital to the community and to reduce re-admissions to the ED/hospital. Support includes ensuring patient has housing, follow-up physician and ancillary appointments, arranges transportation to medical appointments if needed, and is receiving the equipment and services prescribed in the patients discharge plan. This will be accomplished by assisting families in accessing community and personal resources that will enable them to have successful living experiences. Serves as a link between organizations serving the homeless and the Medical Center to improve coordination of services. Represents hospital on Homeless Task Forces. Supervises service learning and social work interns.

Required Qualifications:

  • Master's Degree in Social Work from an accredited School of Social Work, or in Psychology, Sociology, Gerontology, Human Services or a related field from an accredited university.
  • 1 year experience working with the unhoused population. Familiarity with accessing community resources (i.e., housing, supportive services, mainstream benefits, etc.).

Preferred Qualifications:

  • Experience working directly with populations with substance abuse disorders and addictions, including MAT programs, knowledge if harm reduction philosophies and treatment plans.
  • Experience working directly with populations experiencing mental health crisis or episodes.
  • Ability to identify and administer appropriate, brief screening measures to assist with diagnostic understanding and treatment / discharge planning options, including understanding of the DSM – 5.
  • Experience or knowledge of brief, structured intervention techniques (motivational interviewing, CBT, etc.).
  • Experience with Crisis intervention.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
