Social Worker (MSW) -- Home Health

Part Time / PRN -- Omaha, Nebraska

As a Home Health Social Worker, you'll play a vital role in supporting patients and families through education, advocacy, and compassionate care. You'll collaborate closely with a skilled interdisciplinary team to ensure every patient receives the highest level of support.

What We're Looking For

• Master of Social Work (MSW)
• Strong personal integrity and a caring, patient-centered approach
• Ability to build positive relationships with patients, families, and team members
• Excellent communication and problem-solving skills
• Experience in home health preferred, but not required
