Schedule: Exempt (salary), 40 hours per week | Monday - Friday, 8am - 4:30pm | No on-call

Home visits are a regular part of this position.
What You’ll Do:
As a Social Worker with Thome PACE, you’ll make a meaningful difference in the lives of older adults by providing compassionate, person-centered case management, counseling, advocacy, and support. As a valued member of the Interdisciplinary Team, you’ll collaborate with healthcare professionals to assess participants, develop individualized care plans, and connect them with resources that promote independence and quality of life. Key Responsibilities
- Maintains accurate and up-to-date case management records, including regular reassessments and documentation of all participant interactions.
- Conducts home visits to private residences of Thome PACE participants as needed. Regular travel is required to meet with participants in their homes, community settings, and external agencies.
- Ensures that all documentation is completed in a timely manner within the Electronic Health Record (EHR) system.
- Actively participates in the Interdisciplinary Team (IDT).
- Coordinates and monitors 24-hour care delivery to ensure that all participant needs are met consistently.
- Provides individual family counseling and leads group counseling sessions to support participants and their families in coping with social, emotional, and health challenges.
- Refers participants and families to appropriate community agencies, services, or facilities.
- Advises staff on the relationship between social, emotional, and cultural factors to medical care and the availability of relevant social services in the community.
- Actively participates in the development of social work policies and procedures to improve the social work component of the program.
- Facilitates the enrollment and disenrollment process for participants.
- Assists participants and families with the Medicaid application and redetermination process through the Michigan Department of Human Services.
- Accurately documents participant changes, interventions, and progress in the medical record in accordance with program policies and regulatory requirements.

Skills, Knowledge and Expertise
- Master's degree in Social Work from an accredited School of Social Work.
- Current licensure as a Social Worker in the State of Michigan.
- Minimum of two (2) years of experience in a health-related field, particularly working with the elderly or frail population, desired.
- CPR certification or willingness to obtain within 90 days of employment.
- Valid drivers license & reliable transportation.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint).
- Strong communication skills, both verbal and written, with the ability to interact effectively with participants, families,and interdisciplinary team members.
- Ability to multi-task in a fast-paced environment while managing multiple priorities effectively.
- Experience with Electronic Health Records (EHR) and documentation in a clinical setting, desired.
