Social Workers help communities get access to the health or social services that they need, improving or providing those services where they are not already in place. Theyparticipatein legislation and help families and individuals get access to counseling and psychotherapy when necessary. Social Workers are essential social, economic voices and advocates who work on behalf of those facing physical and mental illness, disability, divorce, loss, unemployment, abuse,addictionand other daunting life challenges. In this role, you will complete comprehensive, culturally competent assessments including consideration of the relevant biomedical,psychosocialand spiritual factors of the patient and family's needs, thus leading to the implementation of theappropriate socialservice interventions and continuing care options. A caregiver in this position works days from 8:00AM – 4:30PM, with weekend and holiday requirements.

A caregiver who excels in this role will:

  • Demonstrate the knowledge and ability to care for age specific needs of the population served as measured by completion of mandatory training and direct observation by supervisor.

  • Demonstrate an understanding of illness/recovery and the impact on patients and families.

  • Respond to campus wide referrals on the same working day.

  • Identifysignificant psychosocial issues and implementappropriate socialservice interventions and continuing care options through the process of data collection, dataanalysisand implementation of a care plan based on identified needs.

  • Utilize a multidisciplinary approach to ensure continuity of care and comprehensive patient services to maximize the effectiveness of social service interventions.

  • Develop and sustain positive working relationships with patients, families, physicians,nursesand ancillary personnel to promote open communications.

  • Act as a resource to patients, families,physiciansand caregiversregardinginternal/external resources.

  • Initiate team conferences tofacilitatediscussion/consensusregardingcomplex cases.

  • Use crisis intervention techniques to calm,comfortand stabilize patient/families under stress.

  • Utilize knowledge to coordinate realistic referrals and continuing care plans to meet the needs,ageand health status of the patient.

  • Inform the patients,familiesand anyone accountable for the continuing care of the patient of their role and responsibilities relating to the continuing care plan.

  • Utilizeappropriate clinicalintervention skills toassistpatients,familiesand caregivers with their abilities to cope based on assessed needs.

  • Document on CM progress notes, multidisciplinary problemlistand interdisciplinary education records to provide clear, concise record of social work services and interventions.

  • Facilitate/coordinatetimelytransfer and discharge of patients with complex needs.

  • Facilitate/coordinate inpatient and outpatient dialysis arrangements.

  • Disclose medical errors to patients and familiesin accordance withexisting patient safetyplanand disclosure policy, in collaboration with the clinical and medical staff.

  • Assistin developing andmaintainingthe electronic case management resource manual.

  • Attend torepresentCleveland Clinic at the Broward County Baker Act Taskforcemeeting.

Minimum qualifications for the ideal future caregiver include:

  • Master's Degree in Social Work

  • One year of clinical experience

  • Basic computer skills with familiarity with Windows based software programs

  • Experience in crisis intervention, psychosocial and therapeutic counseling

  • Experience inutilizationmanagement, managed care,qualityand payor issues

  • Strong organizational and prioritization skills anddemonstratedleadership skills

Preferred qualifications for the ideal future caregiver include:

  • State Licensed Clinical Social Worker (LCSW)

  • Two years of experience in hospital/healthcare Social Work

  • Clinical experience working in a hospital environment

  • Knowledge of acute care regulatory/accreditation requirements

Physical Requirements:

  • Requires frequent walking from department to department.
  • Requires sitting at a workstation or desk.
  • Requires standing.
  • Work may include occasional publishing and/or pulling, lifting and carrying objects weighing up to 20 pounds, such as files, documents and computer printouts.
  • Work requires finger dexterity and eye/hand coordination to operate a computer keyboard at a moderate skill level.

  • Follows Standard Precautions using personal protective equipment as required for procedures.
