Comunilife Inc. is looking for a Social Worker-Mental Health Counselor for its residential program in the Bronx, La Central.  The Social Worker-Mental Health Counselor provides supportive services for clients at La Central Residence. Responsibilities include completing mental health and psychosocial assesments, collaborating in the development and implementation of service plan(s), and providing culturally competent and empathetic direct services that support the client’s recovery, vocational, interpersonal goals.  The positions offers a generous benefit package including health insurance, dental, vision, dependent care, paid time off, 401(k), tuition assistance program and more. 
Essential Duties:
  • Obtain Consent from tenants connected to behavioral healthcare treatment and maintain collateral contacts with the goal of continued collaboration with providers.  
  • Complete Initial psychosocial and administer appropriate screenings based on documented diagnosis within 30 days of placement.  
  • Update psychosocial status on a yearly basis following initial assessment.  For tenants connected to care, request an updated yearly psychosocial from their providers.
  • Upon completion of psychosocials, the MHC will develop service plan goals based on psychosocial recommendations.  
  • Coordinate follow up and monitor progress towards goals during office/home visits.
  • Conduct hospital visits to advocate for the tenants emotional health and take part in interdisciplinary discharge care meetings to ensure the tenant receives the appropriate level of care upon discharge back to the community.  
  • Make appropriate interventions when a tenant is emotionally distressed and requires immediate medical care.  Document hospitalization in AWARDS and follow up by documenting all progress made.
  • Follow up with all in-house referrals made from La Central
  • Take part in incident reports, case conferences, service plans and High Risk meetings and ensure all documented work is ready to be presented as scheduled.
  • Take part in the referral process for the NYC 15-15 program which involves reviewing psychosocial and psychiatric evaluations to determine eligibility criteria.  Interview prospective tenants and administer screenings as necessary depending on level of need.
  • Monitor compliance with psychotropic medications and  assist tenants in need of assistance obtaining their medications from their assigned pharmacy.
  • Provide individual and group mental health counseling services to the clients of La Central Residence
  • Document all services provided in a timely manner in accordance with Comunilife, funder, and regulatory requirements
  • Maintain any/all required credentials and keep current with appropriate evidence-based and other clinical practices
  • Work collaboratively in an interdisciplinary team to ensure comprehensive, client-centered services are delivered and documented 
  • Responsible for the Behavioral Health portion of the Face Sheet in AWARDS.  Responsible for updating psychotropic medications and appropriate providers.



Qualifications

Education and Experience Requirement(s):
  • Master’s Degree in Social Work or Mental Health Counseling 
  • Two (2) years’ work experience 
  • Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families a plus  
  • Bilingual skills in English/Spanish a plus
  • Knowledge and hands-on experience in providing direct services related to the treatment and recovery of persons with HIV/AIDS, substance abuse, serious mental illness, and homelessness
  • Excellent oral and written communication skills; proficiency with MS Office and Google product suites
  • Ability to perform and manage in a team approach; excellent interpersonal skills 
  • Ability to handle complex challenges and balance multiple demands on time and attention
  • Ability and willingness to perform all duties as workload necessitates
