Job Description

Liquid Personnel is recruiting an Adult Social Worker to join its client’s Learning Disability Team, based in Rhondda Cynon Taf. The role involves supporting adults with learning disabilities, focusing on reassessments and statutory reviews, working collaboratively to deliver person centred interventions and achieve outcomes within a supportive team environment.

What will your responsibilities be?

The successful candidate will play a key role in supporting adults with learning disabilities, with a primary focus on reassessments and statutory reviews. You will work in a collaborative environment, contributing to positive outcomes for individuals through high quality, person centred social work practice.

  • Safeguard and promote the wellbeing of adults through effective assessment, care planning, and review
  • Undertake assessments in line with the Social Services and Well being (Wales) Act 2014
  • Identify personal outcomes and develop or review care and support plans as required
  • Assess and manage risk in a balanced and proportionate way that promotes independence, choice, and co production
  • Build on individual strengths to achieve positive and sustainable outcomes
  • Work collaboratively with professionals and external agencies as part of a multi disciplinary approach
  • Maintain accurate and up to date electronic records, including case notes, assessments, and reviews, in line with departmental policies

  • Community based role with travel required across the service area
  • Hybrid working model with one office based day per week and flexibility for additional office attendance if preferred

Qualifications and Experience:

To be successful in this role you must have;

  • Qualified Social Worker with relevant registration
  • Proven experience within adult services, ideally within learning disability teams
  • Strong assessment and review experience
  • Ability to work independently and as part of a team
