Job Description

Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Learning Disabilities team, based in Havering.

What will your responsibilities be?

  • Manage a diverse caseload of adults with learning disabilities, including complex cases depending on experience.
  • Work closely with adults, carers, healthcare professionals, and community partners to deliver positive and lasting outcomes.
  • Undertake person-centred assessments, reviews, and support planning.
  • Complete safeguarding enquiries and risk assessments to protect vulnerable adults.
  • Promote independence and ensure individuals receive the right support at the right time.
  • Support safe hospital discharges and work to prevent avoidable admissions.
  • Develop and review personalised care and support plans.
  • Work collaboratively within a multi-disciplinary team alongside health and social care professionals.
  • Maintain accurate and timely case records and reports.
  • Contribute to service development and continuous improvement initiatives.
  • Provide mentoring or supervision support to colleagues where appropriate.

Qualifications and Experience:

To be successful in this role you must have;

  • A recognised Social Work qualification
  • Current Social Work England (SWE) registration
  • Experience undertaking assessments, reviews, and safeguarding interventions within adult social care
  • Good knowledge of the Mental Capacity Act and relevant legislation
  • Strong multi-agency partnership working skills
  • Excellent communication, organisational, and problem-solving abilities
  • A compassionate, proactive, and person-centred approach to practice
  • A commitment to delivering high-quality outcomes for adults and their families
