Job Description

Liquid Personnel is recruiting an Adult Social Workers to join a dedicated Learning Disabilities Review Project in Taunton, for our client. This is an excellent opportunity to work within a supportive multi-disciplinary environment, undertaking Care Act reviews and reassessments to ensure adults receive the right support and achieve positive outcomes.

What will your responsibilities be?

You will be responsible for carrying out comprehensive Care Act reviews and reassessments for adults with learning disabilities across Somerset. The role involves face-to-face visits, strengths-based assessments, evidence-based decision making and close partnership working with health colleagues, providers, families and other stakeholders.

  • Undertake Care Act reviews and reassessments.
  • Complete face-to-face visits in community, supported living and residential settings.
  • Produce high-quality, evidence-based assessments, reviews and support plans.
  • Identify and evidence eligible care and support needs.
  • Contribute to Continuing Healthcare (CHC) processes and funding discussions.
  • Work collaboratively with multidisciplinary teams and partner agencies.
  • Maintain accurate and timely case records.
  • Manage your own workload and deliver outcomes within agreed timescales.

Working Arrangements:

This is a hybrid role requiring attendance in a Somerset office a minimum of two days per week, with travel across the county to attend visits and assessments.

Qualifications and Experience:

To be successful in this role you must have;

  • Qualified Social Worker with a recognised Social Work degree or equivalent.
  • Current Social Work England registration.
  • Significant post-qualified Adult Social Care experience.
  • Strong background in Learning Disabilities services.
  • Experience completing Care Act reviews and reassessments.
  • Excellent assessment, analytical and report-writing skills.
  • Knowledge of CHC processes, joint funding arrangements and health/social care boundaries.
  • Ability to work independently and make defensible professional decisions.
  • Full UK driving licence and access to a vehicle.
