  • Ensuring timely, accurate case recording and contributing to a culture of learning and reflective practice

About You

You will be a registered Social Worker who is passionate about empowering people and promoting independence. You will bring a strengths‑based, person‑centred approach and be confident working within a multi‑disciplinary environment.

You will be able to demonstrate:

  • A commitment to equality, diversity, and inclusion
  • Excellent communication, assessment, and organisational skills
  • The ability to manage competing demands while keeping residents at the centre of decision‑making

About the Team

Intermediate Care is an innovative, inclusive, and supportive service operating across five local areas within the borough. Working in small multi‑disciplinary groups enables the team to build strong community links, understand local needs, and respond effectively to diverse communities.
