As a Primary Care at Home Social Worker, you will play a vital role in supporting patients with chronic and complex medical needs in the comfort of their own homes. This position provides compassionate counseling, care planning, and ongoing support while addressing social determinants of health and helping patients and families navigate challenges.
Working closely with an interdisciplinary care team, you will promote patient stability and independence, support care transitions, and connect individuals to community resources and services that enhance quality of life. Duties / Responsibilities:
Senior Social Workers in the Primary Care at Home program play a critical role in supporting patients and families by identifying social, emotional, and environmental needs and connecting them with appropriate resources and services. This role provides a full spectrum of social work and discharge planning services, delivering comprehensive case management for patients with complex medical, geriatric, or behavioral health needs. As a key member of the care team, the Senior Social Worker helps ensure patients receive coordinated, compassionate support that promotes stability, independence, and overall well-being.
Position Qualifications:
Master’s degree in social work (MSW) from an accredited graduate school of Social Work
Minimum of 1-2 years of related work experience preferred.
Delaware Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) preferred.

Hourly Pay Range: $30.34 - $48.55This pay rate/range represents ChristianaCare’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
