Liquid Personnel are proud to be supporting Medway Council in recruiting a Social Worker to join their Hospital Integrated & Community Discharge Team on a permanent basis.

Please note: This position requires the successful candidate to be on-site 5 days per week

About the Role

As part of the Hospital Integrated & Community Discharge Team, you will support adults through the discharge process, helping to ensure safe and timely transitions from hospital settings into the community.

Key responsibilities include:

  • Working across acute hospitals, non-acute hospitals and community settings
  • Supporting the delivery of the Discharge to Assess model to ensure effective and timely hospital discharges
  • Completing strengths-based assessments and care planning to promote independence and wellbeing
  • Collaborating closely with health professionals, council colleagues and partner agencies to achieve person-centred outcomes
  • Ensuring available resources are utilised effectively to meet assessed needs
  • Working in accordance with relevant legislation including the Care Act, Mental Capacity Act and Mental Health Act
  • Maintaining high professional standards while delivering effective social work intervention
  • Promoting safeguarding and ensuring the welfare and protection of vulnerable adults at all times

About You

To be considered for this position, you will have:

  • A recognised Social Work qualification (Degree, DipSW, CQSW or equivalent)
  • Registration with Social Work England
  • A minimum of 2 years' post-qualifying Social Work experience
  • Experience within Adult Social Care, hospital discharge or community-based services
  • Sound knowledge of the Care Act, Mental Capacity Act and Mental Health Act
  • Strong assessment, communication and partnership-working skills
  • A commitment to person-centred and strengths-based practice
  • A full UK driving licence and access to your own vehicle
