Social Worker - Hospice - PRN

Essential Responsibilities:

Responsibilities listed in this section are core to the position. Inability to perform these responsibilities with or without an accommodation may result in disqualification from the position.

  • Systematically gathers pertinent psychosocial data for patients whereby patient and family psychosocial needs can be identified and appropriate interventions performed. Activities include, conducting home visits to perform a psychosocial assessment for the patient and family, incorporating social, cultural/ethnic, emotional and spiritual aspects of care.
  • Develops psychosocial plan of intervention utilizing all available sources of information. Activities include developing a focused plan of intervention based upon the psychosocial assessment in collaboration with the interdisciplinary team when appropriate and based upon the patient identified needs. Incorporates social, emotional and spiritual aspects of care into the patient’s plan. Performs assessment/data collection in an ongoing and systematic manner, focusing on psychosocial, self-care, education, and environmental factors.
  • Uses pertinent performance metrics to drive improvements in practice that will enhance patient or consumer safety, satisfaction, efficient and effective care and that will identify and promote best practices and equitable care on a multidisciplinary basis.
  • Collaborates with physician, patient/family and other assigned disciplines regarding care needs, formation of and revisions to plan of care.
  • Treats patients and their families with respect and dignity. Advocates for needs, decisions, and rights of patients and families.
  • Provides liaison function between outreach programs and hospice, home health, long term care facilities and other community services as needed.
  • Facilitates advance care planning. Activities include addressing end of life issues, healthcare proxies, living wills, advance directives, and power of attorney.
  • Stays abreast of changing care management and Social Work clinical trends, regulatory matters and payer requirements.
  • Establishes and maintains positive working relationships with patients, family members, co-workers, physicians and referral sources.
  • Completes timely and accurate documentation as per agency policy.
  • Act in accordance with the established mission, vision, and values.
  • Abide by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).
  • Maintain effective communication and professional working relationships with patients/clients and their representatives, team members, contractors, physicians, peers, outside agencies, and the public.

Minimum Qualifications

Required Education and Experience

  • Completion of an accredited BSW program.

Required License/Certifications/Registrations

  • Current Social Work license in the State of Kansas.
  • Healthcare Provider BLS – if BLS is not American Heart Association or American Red Cross, BLS is required within three months of hire
  • Valid Kansas State Driver’s License
  • Auto Insurance equal to or greater than the Kansas insurance requirements.

Preferred Qualifications:

Preferred Education and Experience

  • 1-3 years’ experience in medical facility, hospice and/or home care
  • CHP-SW
  • Completion of an accredited MSW program.

Preferred License/Certifications/Registrations

  • None

Knowledge, Skills, and Abilities

  • Social Work assessment and counseling skills
  • Knowledge of state and federal programs that provide medical care and financial support.

With or without accommodation.

  • Medium Work: Occasionally exerting up to 50 lbs - constantly exerting up to 10 lbs. 26-50% of the day may be standing or walking.
